File a complaint with the public prosecutor

1. Complete the various sections
2. Download your complaint in PDF format
3. Print and sign your complaint
4. Send the complaint by simple letter or RAR: titleContent to the public prosecutor territorially competent. This is the prosecutor of the court of the place of commission of the offense or of the domicile of the offender.

Attach to your complaint all your evidence documents. For example: medical certificates, work stoppages, photographs, videos, miscellaneous invoices, findings.
