Complementary health and solidarity: simplified allocation for certain beneficiaries
The Social Security Financing Act for 2024 provides for a simplified allocation and an automatic renewal of the Solidarity Health Complementary (C2S) for part of those receiving the specific solidarity allowance (SSA) and the youth employment contract allowance (YCA).
An Order in Council will set the exact date for the coming into force of these amendments. In the meantime, the information presented on this page remains up to date.
Do you have low resources? The complementary solidarity health (C2S) helps you with your health expenses. It replaces supplementary universal health coverage (CMU-C). It allows you to get reimbursed the additional part of your health expenses. This means that this aid covers what is not reimbursed by the Health Insurance. C2S is free or paid for depending on your income. We explain the rules in metropolis and in the Dom.

Complementary health solidarity (C2S): free or paid?
Title: Complementary health solidarity (C2S): free or paid?
C2S is a mutual society to help low-income households pay their health costs. It complements social security repayments.
It can be free or paid (less than €1 per day and per person) according to:
- Your resources (income and support)
- Your age
- The number of people in your household.
Examples based on your resources:
- For an adult, the complementary health solidarity is free for an income equal to or less than €10,399.
It pays for income between 1€0,166 and €13,724. - For an adult and 2 children, the complementary health solidarity is free for an income equal to or less than €18,298.
It is paid for income between €18,298 and €24,703. - For 2 adults and 2 children, the complementary health solidarity is free for incomes equal to or less than €21,348.
It is paid for income between €21,348 and €28,820.
You can move to a higher annual resource band if you are receiving housing benefits or if you are a homeowner.
(Example: a household made up of a person earning less than €10,666 and a recipient of housing assistance shall have its resources increased by a lump sum. They now exceed €10,666 and the C2S becomes profitable).

General case
If you have scarce resources , the solidarity health complementary helps you with your health expenditure.
Thanks to the solidarity health supplement, your health expenses are reimbursed in full, within the limits of the Social Security rates.
Warning
Without this help, you have to pay the complementary share, that is which is not refunded by Health Insurance.
In addition to financial assistance, you are entitled to other benefits:
- No fee overruns
- In case of hospitalization, reimbursement of daily flat rate. This is the daily amount you normally have to pay for accommodation and food
- No medical deductible or flat-rate participation by €2 to be paid
- Third-party payer : you do not have to advance medical expenses on medical rates if you respect your care path. This means going through your doctor first before going to see a specialist
- Reimbursement packages for prostheses dental, glasses, aid auditory
- Reimbursement packages for medical devices such as a cane, one wheelchair or bandages
- Discounts on your train tickets depending on the region (if you have the complementary free solidarity health).
Example :
At the pharmacy, you don't advance money for your medications.
To obtain the complementary health solidarity, your income shall not exceed a certain amount.

If you are receiving active solidarity income (SSA), you are entitled to the solidarity health supplement free. This assignment is automatic unless you object.
ASI
Since 1er july 2024, where theASI is allocated to you, your health insurance fund systematically offers you C2S for a fee. This proposal is made by mail.
This presumption does not apply if the holder of the Asi and, possibly, his spouse, cohabiting partner to whom he is connected by a Civil partnerships: titleContenthas been employed or self-employed during the three calendar months prior to the submission of their application for the solidarity health supplement.
Aspa
Solidarity allowance for the elderly
You and your spouse have easy access to paid C2S provided that you have not worked in the 3 months preceding your C2S application.
AAH
You are entitled to the C2S fee if you:
- Live alone and without dependent children
- Have not been in employment for 3 months prior to the date of application for C2S
- Receive the maximum amount of AAH or you receive it in addition to an invalidity pension, retirement pension or pension.
The rules differ depending on whether it is a 1re request or not.

The resources taken into account are those collected during a 12-month period. This period runs from the 13the month to 2e calendar month preceding the month of the application.
Example :
You applied on February 15, 2025. The month of application is therefore February. The period considered (between the 13the and 2e month before) is therefore the period from 1er January 2024 to December 31, 2024.
If you have income from investments subject to income tax (savings account, bank savings book, futures account, Perp...), the income taken into account is that of the last known tax notice.
If you get a housing aid, one lump sum is added to your resources.

You must meet the following 2 conditions:
- Have a valid residence permit to stay in France
- Reside in France continuously for more than 3 months.
If you are homeless, you can get a administrative address with a communal social action center (CCAS) or an approved association.

In France for less than 3 months
You must be in one of the following situations:
- You benefit from a compulsory social security scheme because you have a professional activity in France that lasts more than 3 months
- You are enrolled in an educational institution or an internship in France under cooperation agreements or you are enrolled in a vocational training course for a period of more than 3 months
- You receive certain benefits (family benefits, allowances for the elderly, housing, social assistance, replacement income, allowance for disabled adults, etc.)
- You have volunteered internationally abroad and are not entitled to health insurance in any other way.

You must fill out the cerfa n°12504 form and send it to your health insurance body.
Request for complementary health solidarity

- Local Housing Tax Notice
- If you have lived abroad in the last 12 months: proof of the tax and social situation of each country concerned

You have an Ameli account
If you cannot move, it is possible to give a power of attorney to a middle-aged man.

Your representative must go to one of the reception points of the CPAM: titleContent with the following documents:

Your health insurance body give you his decision no later than 2 months after receiving your request. If you don't get an answer for more than 2 months after your request, it means your request is accepted.

If you've received a refusal, you can challenge it. You have 2 months from the day you received the refusal (notification) to apply to the Amicable Appeals Board (CRA).
