Occupational medicine: what is enhanced individual monitoring of an employee?

An employee exposed to particular risks for his health or safety, for those of his colleagues or third parties, benefits from enhanced individual monitoring (IRS) of his health status. What are the conditions for this follow-up? We're taking stock of the regulations.

The conditions for enhanced individual monitoring differ depending on whether the employee is a worker seasonal, one acting or no. Workers agricultural are also affected by these provisions.
General case
Enhanced individual monitoring of the employee's health status is implemented according to the specific risks to which he is or is not exposed.
Enhanced individual monitoring (IRS) concerns employees exposed to particular risks to their health and safety.
The CRS includes a pre-employment fitness medical examination performed by the occupational physician. This review replaces the information and prevention visit (Vip).
This medical examination of aptitude is renewed at least every 4 years. The occupational physician may choose a shorter periodicity for its renewal.
One intermediate visit is performed by a health professional no later than 2 years after the visit with the occupational physician.
When employees' exposure to these particular risks ceases, or at the latest upon retirement, they also undergo a medical examination. It makes it possible to establish a traceability and an inventory of the exposures to which the employee has been subjected.
These medical examinations make it possible in particular to ensure that the employee is fit for work to which the employer intends to assign him.
They also make it possible to determine whether he is not suffering from a condition that is dangerous for other employees. They also make it possible propose adaptations of the position or assignment to other positions.
Finally, they allow forinforming about risks related to his workstation and the necessary medical follow-up and raising awareness on prevention methods to be implemented.
Employees assigned to positions exposing them to particular risks, as defined by law
These are employees assigned to positions exposing them to the special risks following:
- Asbestos
- Lead according to occupational exposure values
- Agents that are carcinogenic or may cause genetic mutations or impair reproductive function (agents CMR: titleContent)
- Ionizing radiation
- Biological agents of groups 3 and 4
- Hyperbaric
- Fall in height during scaffolding assembly and disassembly operations.
This also applies to employees assigned to positions subject to a specific aptitude test. These include young people under the age of 18 who are assigned to prohibited work for which derogations have been granted.
Please note
In the agricultural sector, other work This monitoring is also necessary (work in a controlled atmosphere for the conservation of foodstuffs, work involving repetitive gestures at high rates, for example).
Employees assigned to positions exposing them to particular risks, as defined by the employer
These are the employees who may be affected by the list of positions posing particular risks to their health and safety.
The employer completes this list, after consulting the occupational physician and the CSE: titleContent, consistent with the risk assessment and the company sheet.
The employer must justify in writing the inclusion of any position on this list.
This list is updated annually.
It shall be transmitted to the occupational health and prevention service and shall be made available to the prevention services of the social security bodies and the Dreets: titleContent.
Yes, the exemption applies if the worker has medical examination of aptitude in the 2 years prior to hiring. All of the following conditions must also be assembled:
- Identical use with equivalent exposure risks
- Occupational physician in possession of the employee's last aptitude notice
- No notice of incapacity of the employee in the last 2 years
- No individual measures to transform the employee's workstation or to adjust his working time during the last 2 years.
During the medical examination of fitness, a medical record in occupational health is compiled by the occupational physician.
After the medical examination of aptitude (initial as periodical), the occupational physician delivers an aptitude opinion (or incapacity) to the employee.
The notice is annexed to the employee's occupational health record.
If he deems it necessary, the occupational physician may propose to the employer individual measures such as a transfer or a change of position.
The employer shall take the proposals of the occupational physician into consideration and, in the event of refusal, shall state its reasons for doing so.
Some employees are examined by the occupational physician in the following situations:
- Employees receiving or having received enhanced individual monitoring of their health status during their career
- Employees who were exposed to one or more of the particular risks before the implementation of the enhanced individual monitoring system.
The employer requests this visit from the prevention and occupational health service to which it adheres. He immediately informs the employee.
This visit takes place under the following conditions:
- As soon as possible after the end of the exhibition of the employee to particular risks to his health or safety
- Before the employee's voluntary departure or retirement.
An employee who considers that he/she can benefit from this visit and who has not been informed of it by his/her employer may request the organization of this visit directly from the prevention and health at work department.
The employee makes the request during the month preceding the end date of the exposure to risks or during the month preceding his departure from the company.
This request must be made no later than 6 months after the end of this exposure.
The employee informs the employer of his/her approach.
This examination establishes a summary balance and one tracking exposures to one or more occupational risk factors to which the employee has been subject.
This assessment is drawn up on the basis of the information contained in the employee's occupational health medical file, his declarations and those of his successive employers.
The occupational physician shall set up special medical supervision if he finds that the employee has been exposed to dangerous risks, including chemical risks, during his career.
This monitoring of the employee says post-exposure or post-professional is done in conjunction with the attending physician and the medical officer of the social security agencies.
It takes into account the nature of the risk to which the employee has been exposed, the health status and age of the person concerned.
The occupational physician informs the employee of the steps to be taken to benefit from this follow-up.
Seasonal
Seasonal employees exposed to particular risks to their health or safety or those of their colleagues benefit from enhanced individual monitoring (IRS).
