The Cerfa form n°16146 must be completed online, printed and sent (or deposited) to the competent legal aid office.

The documents to be attached to the file are indicated in the accompanying leaflet.

The package leaflet also contains information on the statement of non-coverage by the insurer. This certificate must be completed by you and your insurer if the latter does not cover the costs of the trial and attached to your file.
