Declaration for the distribution among parents of the quarters of pension insurance granted for each child born or adopted on or after 1 January 2010

1. Print and complete the form (1 form per child). If you choose by mutual agreement the beneficiary or the distribution between you of the education and/or adoption surcharges, the form must be completed and signed by the 2 parents. In case of disagreement, only the parent who expresses disagreement must complete and sign the form.
2. Attach the requested supporting documents
3. Send the completed form and the supporting documents requested to the competent pension fund as indicated on page 4 of the form

• Copy of the identity card, passport, residence permit or receipt for application for a residence permit or any other proof of civil status and nationality of each parent
• Copy of the up-to-date family record book or birth certificate with filiation or any document showing the filiation of the child
• Copy of the adoption act or judgment if the child is adopted

And in case of disagreement:

To justify raising the child for more than 2 years:

• If you have taken parental leave: copy of your employment contract mentioning its modification or certificate of affiliation to the old-age insurance of beneficiaries of certain family benefits (AVPF)
  If you have interrupted or reduced your activity: proof of your cessation or reduction of activity (employment contracts, pay slips, tax assessments, affiliation with the AVPF)
• If the child has resided in your household: copy of your divorce judgment providing for sole custody of the child or your tax notice mentioning the number of tax shares.

To justify the reception and adoption procedures as main:

• If you have taken adoption leave: proof of payment of the daily allowance for this period
• If you have taken the adoption steps: presentation of your passport and visas from the child's country of origin in case of adoption abroad
