This form allows you to apply for a reversion of the pension and the additional pension following the death of a government official, a magistrate or a military officer pensioner if you have no access to a computer or network or if you do not have your social security number.
If you do not meet one of these 2 conditions, make your reversion request on the free public official website Info-retirement.fr. The online request allows you to one valid application from all basic and supplementary pension schemes to which your deceased contributed.
Warning
If your loved one died while he was active, the application for the reversion of the pension and the additional pension shall be made by means of the cerfa form no. 12231.
