This form allows you to apply for a reversal of the pension and additional retirement following the death of a public servant, a magistrate or a military member retired if you have not accessing a computer or network or if you do not have your social security number.
If you do not meet one of these 2 conditions, make your reversion request on the official free public site Info-retraite.fr. The online application allows to make a single application valid for all basic and supplementary pension schemes to which your deceased has contributed.
Warning
If your loved one died while he was in operation, the application for reversion of the pension and the additional retirement shall be made by means of the cerfa form n°12231.
