This form allows you to apply for a survivor's pension from the State Pension Service (SRE) and the Public Service Additional Pension (RAFP).

Widow, widower or divorced

  • Photocopy of your family record book (or extract from your birth certificate) or photocopy of your national identity card or passport and copy of your spouse's birth certificate and excerpt from your marriage certificate and death certificate or copy of your spouse's death certificate)
  • Your spouse's death certificate (or copy of the death certificate) or copy of the birth certificate of the civil servant, magistrate or member with mention of the death)

Orphan under 21 or disabled over 21

Photocopy of the parents' family record book (or extract from your birth certificate) or photocopy of your national identity card or passport and copy of the birth certificate of the deceased parent)
