2. Address the form, accompanied by the supporting documents requested, at the town hall of your municipality of residence

Supporting documents to be attached to the form:

• Photocopy of european pet passport of your dog justifying its identification at Icad and its valid rabies vaccination
• Certificate of civil liability justifying that you are covered by an insurance contract in case your dog causes damage to a third party
• Photocopy of your dog's European passport justifying its sterilization if it is a dog of 1^re category
• Certificate of fitness obtained after compulsory training on dog education and behavior and accident prevention
• If you are breeder, proof of your certification professional
