Application for legal protection of an adult (family empowerment or judicial protection)

1. Have the detailed medical certificate by a licensed physician: the list of licensed physicians is available on the court of appeal's website, otherwise request the list of physicians from the court
2. Fill the online form
3. Download the form
4. Print the form
5. Date and sign the form
6. Forward the request and the documents to be attached to the guardianship judge : sending by post or depositing at the reception of the court of justice or near the place of residence of the adult to be protected

• Detailed medical certificate (or certificate of deficiency if the doctor could not talk to the middle finger to protect)
• Full copy of the birth certificate of the person to be protected, less than 3 months old
• Copy (double-sided) of the identity document of the person to be protected
• Copy (double-sided) of your ID
• Copy (double-sided) of the identity document of the person who wishes to be a guardian, trustee, authorized person or substitute decision maker
• Information, of which you are aware, on the personal (family, friends) and material (income, savings, real estate, expenses, powers of attorney) situation of the person to be protected
• Letter of acceptance from family members (including for family empowerment)
• At least 2 value notices if you ask the judge for permission to sell real estate
