2. Date and sign the form
3. Attach the necessary documents
4. Forward Folder to the Victims Guarantee Fund

The form must be accompanied by the following documents:

• Copy of ID (national identity card, passport or residence permit)
• Copy of the initial medical certificate and the consolidation medical certificate if you have it
• Supporting documents specific to the costs already incurred following the attack(hospital bill, pharmacy bills, etc.)
• Bank details

The ITGF will review your application and declare it admissible or inadmissible.

If it is admissible, he has to pay you a provision within a time limit of 1 month according to your request.

He's making you a compensation offer within 3 months from the receipt of your complete file.

The offer of compensation may be challenged with the ITGF Compensation Officer.

If you do not find an amicable agreement, you can refer the case to the Judge of Compensation for Victims of Terrorist Acts (Jivat) of the Paris Court of Justice.
