  1. Print fill out the form by hand
  2. Gather the necessary documents
  3. Send the folder in 3 copies by registered letter to the Director of Registry Services of the Judicial Tribunal competent

Please note

The Director of the competent registry office shall be the Director of the place of residence of the child concerned by the application.

Civil status documents:

  • Full copy of the mother's birth certificate or mothers less than 3 months old (or less than 6 months old if issued by a foreign authority)
  • Full copy of father's birth certificate less than 3 months old (or less than 6 months old if issued by a foreign authority)
  • Full copy of the birth certificate of each child concerned by the applicationless than 3 months old (or less than 6 months old if issued by a foreign authority)
  • Where appropriate, full copy of the marriage certificate less than 3 months old (or less than 6 months old if issued by a foreign authority) or family booklet

Copy of ID:

  • Copy of mother's identification or mothers (national identity card, passport...)
  • Copy of father's id (national identity card, passport...)
  • Copy of identity documents of each child concerned by the application(national identity card, passport...)

Court decisions or notarial deeds as appropriate:

  • Copy of simple adoption judgment
  • Copy of the early joint recognition

Proof of address of each child concerned by the application: copy of last tax notice or certificate of education

Parents are not summoned to court.

The Director of Court Registry Services notified 1 copy of the joint declaration to each parent, by registered letter, and he keeps one.

Warning  

The copy of the declaration endorsed and dated by the Director of Court Registry Services of the Tribunal is the only original copy. In the event of loss, only a copy may be issued to the requesting parent.
