Declaration of loss and request for duplicate of a lost, destroyed or deteriorated hunting permit

1. Date and sign the pre-filled form cerfa n°13944.
   If you are a minor or adult under guardianship, have the form completed and signed by your legal representative
2. Send the completed form and the additional documents requested to theOFB: titleContent.

Documents to be attached to cerfa form n°13944:

• A standardized identity photograph less than 6 months old (size 35 x 45 mm - in color)
• Duplex copy of your valid national identity card or passport or, if you are a foreigner, your ID
• Proof of payment by credit card or proof of transfer mentioning NAME / DUPLICATE
• Prefectural certificate of initial issuance of hunting permit for any permit issued by a prefecture or sub-prefecture before 1^er September 2009.
