Pregnancy record (or maternity record)
Verified 01 March 2026 - Public Service / Directorate of Legal and Administrative Information (Prime Minister)
You are pregnant and a maternity notebook (also called pregnancy diary) has been given to you? It provides you with detailed information on the course of the period of pregnancy to the weeks following childbirth. It is given to you free of charge by your department at the beginning of pregnancy. The doctor or midwife fills in the important medical elements. Communication between health professionals is facilitated. We present you the information to know.
The maternity health record provides information on:
- The progress of the medical follow-up of your pregnancy
- Your rights (example: maternity leave)
- Your obligations (example: medical visits)
- Existing aid (example: early childhood care benefit).
It facilitates the coordination of the follow-up of your pregnancy by health professionals and social workers who accompany you until childbirth.
The maternity booklet is issued to you free of charge by the services of your department during the 1st prenatal examination mandatory, i.e. before the end of your 3e month of pregnancy.
During each medical examination, the doctor or midwife records his findings and indications on your maternity record. Reports and results can be included.
The book is your property. It is strictly confidential and covered by medical confidentiality.
FYI
Professionals who consult it and write annotations are subject to professional secrecy.
