Diabetes and heart failure: how does Health Insurance ensure your medical attention?

Diabetes and heart failure are chronic diseases that require regular medical supervision to prevent complications. From June onwards, Health Insurance will contact 500,000 people who have seen a GP less than twice in the last 12 months while suffering from one of these two diseases. The goal is to ensure that these people have a proper pathway of care.

If you suffer from diabetes or heart failure, it is recommended that you consult your treating physician at least every 3 months. These regular medical appointments allow for:

  • to adjust your treatment as well as possible;
  • quickly identify any worsening of your health;
  • reduce the risk of emergency hospitalization due to the disease.

These consultations are an opportunity to discuss with your general practitioner the difficulties you may encounter (your treatment, the examinations prescribed, the changes in your life habits necessitated by the disease, etc.).

From June 2025, the Health Insurance will conduct a telephone campaign for people with these two chronic diseases to raise awareness of the importance of these regular medical appointments.

If you suffer from any of these conditions and have consulted your general practitioner less than twice in the last 12 months, you may be contacted by telephone.

The purpose of this call is to make sure your care journey is going smoothly ; if necessary, you will be encouraged to make an appointment as soon as possible with your doctor.

Please note

Your treating doctor will monitor you in coordination with other specialist doctors (depending on your illness and condition: cardiologist, diabetologist, geriatrician, etc.) and other healthcare professionals (nurse, dietitian, etc.).
