  2. Indicate the common address and the date of commencement of cohabitation
  3. Print and sign every 2 the declaration of cohabitation

Some town halls establish a certificate of common life (or cohabitation). If this is the case of your municipality of residence, you can ask your town hall to draw up a certificate (free), bringing him the required supporting documents.
