Application for financial assistance for social and professional integration (Afis)

1. Prepare the necessary documents
2. Download and print the form
3. Fill the form
4. Send the form by post or by mail to the MSA Mayenne-Orne-Sarthe

The application must be accompanied by the following documents:

• Copy of valid identity document or residence permit
• Copy of the decision of the prefect authorizing the engagement in a path out of prostitution
• One bank identity statement (if the person does not have one, he can provide a certificate of domicile election to the association that follows him)
• Identification of each dependent child (if the person is a parent).

After receiving the complete file, the MSA specifies to the person the action that will be given to his request.
