Comment faire ?

  1. Fill in the postal code or municipality. This is the postal code of the court of the family's place of residence or, in the case of separate residence, the place of habitual residence of the child(ren).
    In other cases, the postal code of the place where the person against whom you are initiating the procedure resides
  2. Select the “online application” among the 3 options offered
  3. Fill out the form online and attach supporting documents
  4. Validate your request

Documents to attach mandatory are as follows:

  • Deeds of civil status :
    • Full copy of your birth certificate less than 3 months old
    • Full copy of the birth certificate dated less than 3 months of each child concerned by your request
    • Full copy of your marriage certificate dated less than 3 months or your family record book
  • Court decisions : as appropriate
    • Copy of divorce or legal separation judgment

    • Copy of any other court decision (judgment or order of the court, family judge or children's judge) related to your family situation or the subject of your application
  • Copy of your ID (national identity card, passport, etc.)

Documents to attach according to your request are as follows:

  • Proof of your residence (rent receipt, electricity bill, etc.)
  • Copy of your last tax notice
  • Copy of your last tax return
  • Copy of your last 3 salary slips
  • Copy of the proof of the social benefits you receive
  • Copy of any document justifying the changes in your situation that lead you to apply to the judge
  • Copy of any document concerning your budget
  • Any proof of your expenses and resources (bank statement, invoices…).

Et après ?

You are notified by any means (in particular by electronic means) of the date of the hearing.

The other parent is summoned to the hearing by registered letter with acknowledgement of receipt, at the address you have given.

If you consent to transmission electronically, you will receive notices, receipts, summonses sent by the Registry through this means. The cerfa form n°15414 allows you to make this consent.
