Request form for access by persons born of an MPA to the identity and non-identifying data of the donor third party (Form)

To complete your application, you must attach the following documents to the application form:

  • Copy of an official identity document, issued by a public authority, including your name, first name, date and place of birth, photograph and signature as well as the identification of the authority that issued the document (passport, identity card...)
  • Full Copy of your birth certificate, dated less than three months, and not an extract from that act. You must apply to the city hall of your birthplace.

Warning  

It is recommended to read the information booklet (in Package Leaflet) before completing the form.

After verifying that your application is admissible and complete, the CAPADD: titleContent will acknowledge receipt within 2 months.

CAPADD then proceeds with the investigation of the application, searches for the donor third party and, if the donor is identified, seeks their consent.

However, CAPADD cannot commit to a response time, given the potential difficulty in tracing some donors.
