Request for assistance or supplementary benefits from the National Military Social Security Fund (CNMSS) (Form 15929*02)

Cerfa 15929*02

Allows the recipient of a military disability pension (PMI) to apply for financial assistance to compensate for an out-of-pocket expense related to care or a benefit directly related to an infirmity pensioned by the CNMSS and partially (or not) refundable.

This administrative form, to be completed and signed, must be accompanied by medical form (Form 15931*01)
