Application to the guardianship judge for authorization of an act in the context of a legal administration (Form 15731*03)

2. Gather documents necessary
3. Send Folder the juvenile guardianship judge at the competent judicial court

• Copy of the birth certificate of the parent(s), less than 3 months old
• Copy of the child's birth certificate concerned by the application, less than 3 months old
• Documents depending on the authorization requested (see notice no. 52179#03)

The notice no. 52179#03 outlines the process to be followed.

The judge may render its decision solely on the basis of the application and the documents transmitted.

If they want to hear you or your child beforehand, you are summoned to a hearing. During this hearing, the judge hears your explanations, examines the documents that will be given to him and asks the questions he considers useful.
