Declaration of change of usual name (name of spouse)

The online process currently allows a declaration of change of name to be made to the health insurance fund and the family allowance fund.

1. Specify the pattern change of use
2. Select the organization(s) to be prevented
3. Specify the usage name that you wish to use and your personal information
4. Choose the transmission mode of the request (electronic or postal)
5. Attach documents requested
6. Send request online or generate mail(s) to send yourself

• Copy of family booklet
• In the case of a divorce, copy of the divorce judgment
• If the common name you wish to declare is the name of the former spouse, written authorization from the former spouse or a court decision granting the right to use the former spouse's name

One acknowledgement of the request is sent to the e-mail address provided.

• In case of electronic transmission, your request is automatically addressed to each of the selected instructor services. Each department is then responsible for informing you of the follow-up of your file.
• In the case of transmission by post, print the letters generated at the end of the process, then send them or deposit them with each of the selected instructor services.
