Application to register for the examination and issue a hunting permit

1. Date and sign the pre-filled form cerfa n°13945.
   If you are a minor or adult under guardianship, have the form completed and signed by your legal representative (father, mother, guardian or protection litigation judge)
   Have your doctor complete the certificate stating that you do not have any of the medical conditions or disabilities that make hunting dangerous
2. Send the completed form and the additional documents requested to the departmental hunting federation in which you are registered.

Documents to be attached to cerfa form n°13945:

• A standardized identity photograph less than 6 months old (size 35 x 45 mm - in color)
• Duplex copy of your valid national identity card or passport or, if you are a foreigner, your ID
• Proof of payment by credit card or proof of transfer with the following wording: applicant registration number + NAME and surname of the applicant)
• The following documents relating to the obligations of the national service, if you are French and between 16 and 25 years old:
  
  • Census certificate or certificate of participation in the Defence and citizenship day if you are between 16 and 18 years old
  • Certificate of participation in the Defence and citizenship day or provisional certificate if you have not yet participated in this day if you are between 18 and 25 years old.
