Allows you to make a proxy of vote.

You can:

  • Either fill out the form using the computer and then print it out on 2 sheets (no double-sided)
  • Either print the form on 2 sheets (no double-sided), then fill it by hand (legibly and without scratches).

To complete the form:

  • You must provide your national voter number and the number of the elector responsible for voting for you. You can find out these numbers using this online service.
  • You do not need to complete the administration sections.

Please note

This form also makes it possible to terminate a proxy of vote.
