The pharmaceutical file is a free computer file and created automatically, unless you object. It is consulted and fed by your pharmacist. It lists the medications that have been delivered to you, as well as the treatments and doses in progress. The drugs on the record may have been prescribed by a doctor or purchased freely. We present you the regulations to know.

The pharmaceutical file allows in particular:

  • Secure the dispensation medicines (doctor, pharmacist, hospital) by limiting the risk of drug interactions and redundant treatments
  • Ensuring coordination between health professionals
  • Promote improved immunization coverage.

Please note

The pharmaceutical record should not be confused with My health area (shared medical file)), which is in particular a computerized and secure health record.

Since April 5, 2023, the pharmaceutical dossier shall be created automatically. A mechanism says " of automatic opening unless opposed » shall be implemented. The pharmacist collects your contact information (e-mail or postal address).

You will then receive the information related to this automatic opening:

  • By email
  • Or by hand delivered by the pharmacist, if you have not given your email address.

Once this information has been received (by email or in person), a period of 6 weeks begins during which you can object to the opening of the pharmaceutical file.

Without action on your part, the pharmaceutical file will be automatically opened at the end of this 6-week period.

If your pharmaceutical record has been created before 5 April 2023, the National Council of the Order of Pharmacists informs you individually

  • Maintaining your file
  • And your right to ask for closure.

General scenario

You can object to the opening of your pharmaceutical file within 6 weeks after receiving the email from Cnop: titleContent.

This email is sent to you before opening your file and explains the characteristics of this file.

You can also, if you wish, immediately activate the opening of your pharmaceutical file without waiting for 6 weeks.

Without any action on your part, your pharmaceutical file will be automatically opened at the end of the 6-week period.

You're a non-emancipated minor

Your legal representative may object to the opening of your pharmaceutical file within 6 weeks after sending the email of Cnop: titleContent.

Please note

After having objected to the opening of your pharmaceutical file, your legal representative can subsequently request the opening of your file.

You are under guardianship

Your guardian may object to the opening of your pharmaceutical file within 6 weeks after sending the email of Cnop: titleContent.

Please note

After opposing the opening of your pharmaceutical file, your guardian can then request the opening of your file.

Warning  

If you opposed the opening of the pharmaceutical dossier or if you have requested its closure before 5 April 2023, the National Council of the Order of Pharmacists (Cnop) cannot automatically open a file for you until 3 years after your opposition or closure. Example: you requested the closure of your pharmaceutical file on February 15, 2023, the Cnop can not automatically open a file before February 15, 2026.

At the end of these 3 years, a pharmaceutical file may be automatically opened unless you object again.

This opposition can be made by email or by post by returning a form.

Pharmaceutical record: rights management form

If you object to the opening of your pharmaceutical file, the Cnop: titleContent let you know that it takes this opposition into account. Your file will therefore not be opened.

  • The dates and modalities of dispensation and medical prescription.

  • 23 years for vaccines
  • 3 years for other medicines.

Non-emancipated minor

The pharmaceutical dossier shall contain:

  • Data related to your identity (examples: surname, first name, date and place of birth, social security number)
  • Data relating to the identity of your legal representative and his contact details
  • Information (e.g. characteristics, quantity) related to medicines and vaccines that have been given to you with or without a prescription
  • The dates and modalities of dispensation and medical prescription.

The data is kept for:

  • 5 years for biologic drugs
  • 23 years for vaccines
  • 3 years for other medicines.

Person under guardianship

The pharmaceutical dossier shall contain:

  • Data related to your identity (examples: surname, first name, date and place of birth, social security number)
  • Data relating to the identity of your guardian and his contact details
  • Information (e.g. characteristics, quantity) related to medicines and vaccines that have been given to you with or without a prescription
  • The dates and modalities of dispensation and medical prescription.

The data is kept for:

  • 5 years for biologic drugs
  • 23 years for vaccines
  • 3 years for other medicines.

You, your legal representative or guardian may exercise your right of access to:

  • A health professional (pharmacist, doctor in a health facility, medical biologist). This right then relates to the data of the pharmaceutical file and the traces of the actions carried out in the pharmacy or establishment in which this professional practices
  • From Cnop: titleContent. This right then concerns the data of the pharmaceutical file and all traces of actions (biological drugs, other drugs, vaccines).

When you request access to the content of your DP via the content access form, you will receive a paper copy by registered mail.

You can also directly access the content of your DP using thePharma Dossier application referenced on the service catalog of My Health Area.

Pharma dossier is a free application that mainly allows access to the content of his pharmaceutical record.

Reminder

You can request a new pharmaceutical dossier at any time.

The closing of a DP or opposition to its creation are valid for 10 years. After 10 years, if you have given your email address during a visit to a pharmacy, you will again receive an email indicating the automatic opening of a DP unless opposed within 6 weeks.

If you wish to have a DP before these 10 years, you can apply on rights management portal from the moment your email address is known to the order of pharmacists. In this case, you immediately have a DP.

You can also request it by sending the entitlement management form accompanied by a copy of identification. The creation of your DP will then be effective within a maximum of 30 days after receipt of the request.
